Centralized Credentialing

BayCare’s Centralized Credentials Verification Office (CVO) offers comprehensive health care practitioner credentialing services to BayCare’s facilities.

The CVO complies with the standards of The Joint Commission (TJC), the National Committee for Quality Assurance (NCQA), the Accreditation Association for Ambulatory Health Care (AAAHC), the Center for Medicare and Medicaid Services, the Medical Staff Bylaws, Rules and Regulations and all other applicable regulatory requirements.

CVO Mission

The Credentials Verification Office of the BayCare Health System exists to provide comprehensive credentialing services in an efficient and timely manner to health care practitioners through a thorough review and verification of credentialing information whereby the BayCare Health System Hospital entities can assess a practitioner’s ability to provide high-quality, compassionate patient care. The credentialing process is designed to meet all professional, legal and accreditation requirements by federal and state regulations.

Request for Application Process

To begin credentialing, all providers must complete a Request for Application through a third-party vendor, AppCentral.

Hospital Affiliation Letter

Outside organizations can request verification of practitioners in good standing on the medical staff by accessing iResponse.
