Reappointment is the BayCare CVO process of re-evaluating a practitioner's current competency after they have been appointed to the medical staff or professional staff with a BayCare Health System entity or entities. The BayCare reappointment process aligns with The Joint Commission’s hospital accreditation standards requiring all providers to complete a reappointment process every 24 months for all entities the provider is privileged with.

Reappointment Process

The BayCare CVO will send the provider an email notification via AppCentral with a link to complete the reappointment process. Providers need to complete the reappointment application, crosscheck the demographic information currently in our database for accuracy, complete new attestation questions, review, sign and date the Delineation of Privileges form (if included).
